Kawartha Lakes Haliburton Ontario Health Team (KLH-OHT) is a group of healthcare providers and organizations as well as patient, family and caregiver members working together toward a common goal of improving health outcomes. Our purpose is to have a relentless focus on supporting people to live their healthiest possible life.

Working Groups and Committees
Our working groups and committees are created to support the strategic direction of the Kawartha Lakes Haliburton Ontario Health Team and work to address the healthcare needs of our community.

Oversight
Committee
Develops the strategic plan and budget of the KLH-OHT, oversees the Governance Working Group.

Operating
Committee
Implements plans and design of the KLH-OHT, prepares Ministry submissions, and oversees working groups.

Lived Experience Advisory and Partnership Committee
Advisory group that aims to improve the patient, client, family and caregiver care and service experience in Kawartha Lakes and assist in shaping KLH-OHT programs.

Integrated Care Working Group
Identifying key change initiatives that will advance standardized integrated care for CHF/COPD – optimizing existing resources and preparing for future investments.

Digital and Data
Working Group
Improve ways of sharing patient health records between providers, enhance seamless communication across multiple care settings, increase digital access to patient health records.

Palliative Care
Working Group
Enhancing access to community-based palliative care in alignment with the Palliative Care Health Services Delivery Framework

Frailty and Social Prescribing Working Group
Develops and promotes non-clinical supports, activities, and resources that connect people to their community, improve wellbeing, and address frailty through early identification and coordinated care

Quality Improvement
Working Group
Identifies opportunities to improve healthcare services, implements best practices, and tracks progress to ensure care in our region is safe, effective, and continuously improving

Attachment Strategy
Working Group
Works to ensure every person in our community is connected with a primary care provider, develops strategies to close attachment gaps, and supports equitable access to care
