Progress on Expanding Primary Care Access Across Kawartha Lakes and Haliburton

The Kawartha Lakes Haliburton Ontario Health Team (KLH-OHT) is pleased to share an update on our ongoing efforts on behalf of our partners to strengthen access to primary care across our region. Working in partnership with local Family Health Teams, Nurse Practitioner-Led Clinics, Community Health Centres, and independent physician practices, we continue to make progress in connecting unattached residents with a primary care provider.

As part of this system-wide effort, we are currently working through the Health Care Connect (HCC) waitlist to match patients with new or expanding primary care teams. If you do not have a primary care provider, registering with Health Care Connect is the most important step you can take. You can register online at ontario.ca/healthcareconnect or by calling 1-800-445-1822 .

Importantly, you no longer need to “de-roster” from your existing provider (for example, if you are still rostered to a doctor or nurse practitioner in another community far away). Registering with HCC will not affect your current care but will help us identify who in our local communities is actively seeking a new provider closer to home. This updated process helps ensure that all residents who want to access care locally are counted and prioritized appropriately.

To date, we are pleased to report that:

700 new patients are being attached at the Haliburton Highlands Family Health Organization (approximately 250 to date),
100+ new patients are being attached to Nurse Practitioners and/or Family Physicians practices in Bobcaygeon, Woodville and Lindsay
Various primary care providers across the KLH-OHT geography continue to expand capacity to welcome more patients as we build capacity across the system.

These efforts represent important progress toward building a stronger, more equitable primary care foundation—one that ensures every resident has access to comprehensive, team-based care close to home.

We thank all residents for their patience as we continue this important work and encourage everyone seeking a local provider to register with Health Care Connect today.

Lists are great ways to stay on track. Write down some big things you want to accomplish and some smaller things, too.

Check the list regularly

Don’t forget to check in and see how you’re doing. Just because you don’t achieve the big goals right away doesn’t mean you’re not making progress.

Reward yourself

When you succeed in achieving a goal, be it a big one or a small one, make sure to pat yourself on the back.

Think positively

Positive thinking is a major factor in success. So instead of mulling over things that didn’t go quite right, remind yourself of things that did.

AODA training gives employees/volunteers the basic knowledge about accessibility, and how it influences people with disabilities. We often think of disabilities as either physical or visible. However, AODA includes all disabilities: physical, visible, and non-visible disabilities, such as learning and mental health disabilities.

Employees who take AODA-specific training on customer service learn how to provide excellent customer service to people of all abilities. For example, when interacting with an individual with vision impairment always identify yourself when you approach them. Also speak directly to them, not their service animal or guide. If you need to leave, let the person know. Whereas, if an individual has a mobility disability, you don’t necessarily have to identify yourself, but you should never touch their equipment without permission

Bridges out of Poverty*

Training provides a framework for understanding the challenges faced by individuals living in generational poverty and dives into the complex barriers that can keep them there. The workshop explores the hidden rules of economic class, the resources required for stability, and the importance of building social capital to foster long-term stability. It explores the reality that moving out of poverty requires more than just financial resources—it requires hope, resilience and building healthy and supportive relationships and networks.

This course’s goal is to heighten your comfort level and competency in providing high quality, inclusive, affirming, welcoming, culturally sensitive care. In the course you will build your understanding of important 2SLGBTQ terminology.

Rainbow Health Ontario

Rainbow Health Ontario (RHO) is an online registry operated by the Sherbourne Health Centre in Toronto to help improve healthcare for 2SLGBTQIA+ people. They do not offer health care directly, but do have a huge resource library and a database of queer-friendly or gender-affirming services around Ontario.

RHO publishes guidelines for healthcare practitioners regarding hormone

Why did you join the Kawartha Lakes Haliburton Ontario Health Team's PFAC?

"As a Respiratory Therapist, I retired from the Ross Memorial Hospital on August 31, 2021. I worked for 32 years at the Ross mostly in a part time or casual role. During that time, I worked full-time at SickKids in Toronto, opened a homecare office in Peterborough and worked at the Peterborough and Orillia hospitals.
Often I saw patients coming to the ER at the various hospitals with conditions that could have been properly managed if they had been connected with the appropriate medical professionals. Effective navigation our local health care services by our patient population is one of the important goals of the KLH-OHT Patient and Family Advisory Council. If even one patient has achieved better health or reduced anxiety due to our efforts, then our efforts are invaluable.
By being a PFAC member, I still feel engaged in patient care and continue to be a positive force in alleviating pain and suffering.
The City of Kawartha Lakes is on track to double its population in the next 10 years. Having systems in place that help patients and medical professionals to connect is going to be essential to the growth of our community. National Day For Truth and Reconciliation 2023

September 30th marks the National Day for Truth and Reconciliation. The Kawartha Lakes Haliburton Ontario Health Team recognizes this day and remembers the Indigenous children that tragically lost their lives at residential schools and acknowledges the ongoing effects still experienced today by families, communities and survivors of Canada’s residential school system. The journey of Truth and Reconciliation reinforces the imperative that the health system of the future be grounded in equity, inclusion, respect and compassion.

This day is an opportunity for education and to explore the experiences and culture of First Nations, Inuit and Métis peoples, listen to their stories and reflect on the effects still felt today from these residential schools. Today also coincides with Orange Shirt Day, when all Canadians are encouraged to wear orange to raise awareness of the tragic legacy of Canada’s residential school system.

Within the surrounding Kawartha Lakes and Haliburton County areas, there are a number of events being held to mark the day.

Launch of The East Region Virtual Care Clinic

The Durham Community Health Centre is pleased to announce the launch of The East Region Virtual Care Clinic .

Durham Community Health Centre will serve as the East Region Hub for this Ministry of Health-funded program designed to deliver a regional Nurse Practitioner-led Virtual Care Clinic that works with local health care providers and Ontario Health East to ensure patients can access more timely care. This virtual online clinic is available to all residents of Ontario’s East Region (see map of the East Region below, for reference). This includes residents of Kawartha Lakes and Haliburton County.

East Region Virtual Care Clinic will improve timely access to care, reduce Emergency Department wait times and support patients in receiving care in the comfort of their home, whether it be by video or phone.

The clinic will serve:

Patients with new or persistent cough, fever, rash, or other similar symptoms
Patients with imminent concerns but are not life or limb threatening
Patients with or without a family doctor

How the Clinic will work:

Minden Hills Urgent Care Clinic

The new Minden urgent care clinic provides access to same-day, urgent, non-emergency health care. The clinic runs on a first come, first served basis and is available to any Canadian resident with or without a health card.

The clinic is run by the Kawartha North Family Health Team and is staffed by KNFHT Nurse Practitioners and Registered Practical Nurses who will provide care to patients with unexpected but non-life-threatening health care concerns.

Hours of operation are 10am-5pm seven days per week.

Virtual Services

Health811 is a phone or online resource, open 24 hours a day and available to all Ontarians. Call 811 to find health services or information or get health advice from a registered nurse.

Telus Health MyCare is a virtual service available from 8:00am to 10:00pm Monday to Friday, and 8:00 am to 6:00pm on weekends. You can see a doctor from your phone, and patient visits are covered with a valid OHIP card.

The East Region Virtual Care Clinic delivers a regional Nurse Practitioner-led Virtual Care Clinic for residents of Ontario's East region, which includes Kawartha Lakes and Haliburton County.Patients can access the clinic 7 days a week from 1:00 to 9:00 p.m. a health card is not required.

Enhanced Care is a walk-in tele medicine clinic. There are 2 locations:

Fenlon Falls inside the Fenlon Falls Community Pharmacy at 205 Francis Street East, Monday to Friday 9-3
Lindsay at 189 Kent St West inside the Victorian Drugmart Guardian Pharmacy Monday to Friday 9am to 9pm, Saturday and Sunday 9am to 6pm

Good Doctors Lindsay is an in-person virtual clinic located at 86 Russell St West, Lindsay. There is a nurse on site to perform physical assessments, and connect patients through secured video conference, to a licensed Ontario physician. Physicians are not physically onsite for assessments. You must book through the website, and appointments are available Monday, Wednesday and Friday from 9:00 am to 4:30pm 705-304-0021 .

MD Connected is a virtual care clinic located inside Rexall at 401 Kent St W, Lindsay. Nurse-assisted doctor consultations are available in the clinic, using telemedicine technology. Available Monday to Friday from 9:00am to 7:30pm, Saturday 9-5, and Sunday 10-5. A small fee may apply.

Frequently Asked Questions

Who can attend the clinic?

Only those without a health care provider (doctor or NP) anywhere in Ontario and you live in the City of Kawartha Lakes.

Can I book an appointment in advance?

No, you can book an appointment the same day of the clinic (Fridays) starting at 8:00am to 9:30am by phone or online. Please do not come to the clinic without an appointment OR to book an appointment.

What kind of appointments are available?

This clinic will see patients for non-crisis mental health issues, diabetes, prescription refills (excluding Narcotics), cough, cold, flu like symptoms, urinary tract infections, skin concerns, stitch removal, aches and pains, etc.

Can I get medical forms/notes filled out?

No, unfortuntately the staff at the clinic will not be able to fill out medical forms such as ODSP, hearing aids, WSIB, sick notes.

If I don’t have an OHIP card, can I attend?

No, unfortunately this clinic can only see patients who have an OHIP card. For those without an OHIP card, you can visit the After-Hours Clinic (Monday – Thursday 6pm to 9pm and Saturday & Sunday 9am to 12pm (noon).

City of Kawartha Lakes Family Health Team applauds funding increase by Ontario government to deliver primary care to more patients

KAWARTHA LAKES, ON (February 15, 2023) – City of Kawartha Lakes Family Health Team welcomes an additional investment of $440,000 per year by Ontario’s government. The funding increase announced today by Haliburton-Kawartha Lakes-Brock MPP Laurie Scott will pay for more direct care staff and resources to serve our community.

With this additional funding, City of Kawartha Lakes Family Health Team will be able to recruit two additional nurse practitioners and one additional administrative support member to our growing team. This will enable to us to expand our programs and services to the growing number of unattached patients in the City of Kawartha Lakes.

“On behalf of the board, staff, physicians, and community, we applaud Ontario Ministry of Health’s recognition for support toward team-based primary health care, and specifically, toward the community of Kawartha Lakes where there is a continued need for health care services for the unattached population,” says Aasif Khakoo, Executive Director for the City of Kawartha Lakes Family Health Team. “A very special thank you to MPP Scott for her tireless support to the City of Kawartha Lakes Family Health Team, and the constituents of Haliburton, Kawartha Lakes and Brock. Why did you join the Kawartha Lakes Ontario Health Team's PFAC?

I recently retired from being an active healthcare provider in the community after 52 years as a physiotherapist. I felt my experience and perspective in the field could help in the development of an effective model to improve access to healthcare and the healthcare information highway.

Due to my recent increased need to access healthcare in my community, my vision may be altered somewhat from the “other side of the gurney”. Having orthopedic surgery, contracting Covid, and exacerbating an upper GI condition all at the same time and within 5 days of my partner needing emergency medical intervention does have a revealing impact. For me it has been a “positive” experience but probably made better because it was the right treatment at the right time and made better from my background and ability to navigate the system and ask the questions.

A priority I feel for the KLH-OHT is to educate our populations on the tools available to be able to navigate the system for healthcare in a way that can be accessible to all – print, electronic or personal. As well as to advocate for alternative strategies to provide care.
We need to find a way to provide frontline collaborative services in our individual communities and save the ER for more critical emergency care. We need to put more weight and effort back into Home Care services for wellness and monitoring and healthcare teaching. We need to continue advocating for increased education spaces for all medical practitioners – Doctors, RNs, NPs etc. and have money to support their work. We need to continue to provide the navigation tools and resources to our public so they can use what resources are available but are unknown. We need to use our public resources such as Seniors Centres, churches, libraries to disseminate our work.

Member Spotlight: City of Kawartha Lakes

The City of Kawartha Lakes is a lead partner in the Kawartha Lakes Ontario Health Team. Through the commitment of Council, your local government supports the provincial government in its responsibility to deliver quality health care to residents.

How is the City of Kawartha Lakes improving the patient experience in our community?

1. A quarter of all residents are over the age of 65. To support seniors as they age and develop more complex health needs, Kawartha Lakes Paramedics launched the Community Paramedicine program in 2019. Beginning with one Paramedic in 2019, today there are four full time Community Paramedics and plans are in place to increase that number to eight within two years. The program is funded entirely by the Province and operated by Kawartha Lakes Paramedic Service. The program is based out of the former Oakwood Fire Station and services all of Kawartha Lakes. Sara Johnston, Paramedics Chief, commented, “This program is unique because it allows us to wrap services around the patient. As of November 2022, we’re currently serving an average of 225 patients a week, from Coboconk to Omemee and all points throughout the municipality. We work closely with partners in primary care, home care and Community Care to provide a safety net for patients.”

Marie McAuley, a patient of the Community Paramedic program, shares her story of being cared for by Community Paramedic Julie Milne:

“ I’m 90 years old, am legally blind and live alone. When Julie started checking on me, she discovered that I have a heart condition. She saved my life. I haven’t had a hospital visit in two years. Julie helped me to learn how to monitor myself and the report goes in automatically. If there’s anything irregular, she gets an alert and will call me to see how I’m feeling. When she comes each week, it’s a real bright spot. It saves me going to the doctor, it saves a hospital visit and it makes me feel safe.”

2. Everyone deserves to have a family doctor. As one of the major partners of the Kawartha Lakes Health Care Initiative (KLHCI), the municipality of Kawartha Lakes continues to support recruitment and retention programs for family doctors.

Since the pandemic began, KLHCI has recruited three family doctors and are working with a number of potential recruits. They are supporting physicians to receive ‘hands on’ orthopedic training at the Ross Memorial Hospital, a valued training session that benefits doctors and patients. Why did you join the Kawartha Lakes Ontario Health Team's PFAC?

“I have always been a big believer in two things. First, if you see a problem, you become part of the solution and second, there is power in story. Stories need to be told to create change and sharing those stories with the right people and repeating them creates change.

There have been many gaps and struggles in rural health care. I have seen it first hand in my work and personal life along with the people that are a part of PFAC. We all come with different experiences and we have worked in different fields. It is very frustrating not only for the people that have been trying to navigate the health care system but also for those trying to maneuver themselves and their teams to provide the health care people deserve. Being a part of PFAC has given us hope that there is some change on the horizon, that they are listening and the people who can make decisions are using the information gathered to create solutions that are needed and will work.

The future lies in the continuation of building a simple conduit for information and access that includes real people, not just a machine. We need to find solutions that work to fill in the gaps in regards to mental health by offering training and giving tools to the people who are on the front lines. Having the ambulance drivers trained in Palliative Care is a huge first step in allowing people to die at home, comfortably, with the safety and support they need. Ultimately there needs to be a consistency in care and communication among the health teams that work with us.”

Digital Health Week 2022

November 14 – 20 is Digital Health Week, which is a time to recognize the importance of supporting the digital transformation of health systems across Canada. Digital Health is important now more than ever. It allows for seamless access to health information for patients and providers, and also provides access to important virtual care options.

Kawartha Lakes Ontario Health Team is committed to providing digital health options for the City of Kawartha Lakes. We currently offer our Wellness Connections Program, which helps older adults get connected to local programs and services. The program also provides the option to sign up to our online learning platform, which provides various courses to help older adults learn how to stay healthy at home.

Members of Kawartha Lakes Ontario Health Team are also offering Online Appointment Booking to patients previously registered with participating healthcare providers in the City of Kawartha Lakes. Online appointment booking allows patients to book or change their medical appointments online without the need to call their provider’s office. The Remote Patient Monitoring Program is also available to our community, which enlists the expertise of Community Paramedics to provide health education as well as easy-to-use devices to monitor a patient’s health from the comfort of their home. This program can assist in monitoring various chronic conditions, such as CHF, COPD, hypertension, diabetes, and more. We also continue to work with our member organizations to bring about some exciting digital transformations in how patients within the City of Kawartha Lakes can connect to local services and access their health records. November is Falls Prevention Month in Canada

Did you know?

Falls are the leading cause of injury for older adults across Canada, accounting for over 81% of all injury-related hospitalizations. [1] 4 out of 5 injury hospitalizations involving older adults were because of a fall. [2]

What are the Most Common Causes of Falls?

Slips and trips
Falls from furniture
Falls on stairs

Falls can result in serious injury, loss of mobility, and physical functioning and independence. Fear of falling in itself can be detrimental to one’s overall health and well-being. Many people who have experienced a fall limit or stop doing the things they used to enjoy doing because they are afraid of falling again. This is actually counterintuitive. When we limit or stop being active we lose balance, muscle mass, and independence.

But Wait! There’s Good News!

The good news is that falls are not a normal part of aging and can be prevented.

The Most Effective Ways to Prevent Falls:

Exercise to build strength and balance
Eat healthy food and drink plenty of water
Manage your medications to prevent dizziness
Safeguard your home
Take your time, don’t rush
Wear well-fitted shoes
Use proper eyeglasses and hearing aids
Think about using a mobility device, such as a cane, if needed

New Family Doctors for Community Care City of Kawartha Lakes

City of Kawartha Lakes, November 15, 2022: The Kawartha Lakes Health Care Initiative (KLHCI) and Community Care City of Kawartha Lakes are pleased to announce that Dr. Elizabeth Morrison and Dr. Nivi Navaratnam have joined the Community Health Centre team.

Dr. Morrison received her Doctor of Medicine from the University of Ottawa (2017) and completed her Family Medicine Residency at Queen’s University (Peterborough and Kawartha Lakes site) in 2019. Dr. Morrison has provided locum services to the Community Health Centre over the past few years and is pleased to now join the clinic on a permanent part-time basis. Accepting the part-time position will allow her to continue to provide locum services to family physicians in the City of Kawartha Lakes and City of Peterborough.

Dr. Navaratnam received her Doctor of Medicine from McMaster University (2019) and completed her Family Medicine Residency at the University of Toronto in 2021. Dr. Navaratnam has been providing locum services to the Community Health Centre since August 2021 and is happy to now accept a full-time position. She has accepted the practice of Dr. Allison MacKay.

“Community Care City of Kawartha Lakes is very pleased to have both Dr. Morrison and Dr. Navaratnam join our team”, says Melinda Jayne Gilmour, Director, Clinical Services. Neither physician will be accepting new patients at this time as they will be caring for those that are already part of the Community Health Centre. Anyone wishing to be added to the Community Health Centre’s wait-list is asked to contact the Community Health Centre directly at 705-879-4100 (push 1).

Barb van der Veen, KLHCI Acting President, states that “even through this time of the COVID-19 pandemic, KLHCI is still actively recruiting family doctors for the City of Kawartha Lakes. We are very pleased that both Dr. Morrison and Dr. Navaratnam have accepted positions at the Community Health Centre.”

KLHCI and the Community Care Community Health Centre wish to thank Dr. MacKay for her dedication to her patients over the past two years and wish her well in her future endeavours.

The KLHCI Board of Directors has representation from throughout the City of Kawartha Lakes. The board is interested in adding new members. About the Kawartha North Family Health Team

Kawartha North Family Health Team was first envisioned in 2006 by a concerned group of community members to increase access to Primary Care in Fenelon Falls and area. Originally operating as Fenelon Family Health Team (FHT), we expanded to include Bobcaygeon and other parts of the Northern City of Kawartha Lakes as well as Trent Lakes and Haliburton County and changed our name to Kawartha North FHT to reflect this expansion.

Since our inception we have moved from a total staff of five – consisting of two Nurse Practitioners, one Registered Nurse, one receptionist and an Executive Director to two locations and more than 20 employees. This new compliment includes one part-time and three full-time physicians and a third Nurse Practitioner.

What is KNFHT currently working on to improve the patient experience in our community?

In line with the Kawartha Lakes OHT, Kawartha North has always strived to work collaboratively with our community partners to address patient needs and highlight gaps in the local healthcare environment whilst working to help our patients navigate the health system. To this end, Kawartha North has brought services such as bloodwork and local walk-in clinics to the communities despite a lack of funding, partnered with the Coboconk Norland and Area Chamber of Commerce in the Summit Wellness Centre development and encouraged providers to bring their services to outlying communities to save patients traveling to Lindsay.

Our Member Spotlight series introduces the member organizations that have partnered with the Kawartha Lakes Ontario Health team to improve the local healthcare system for our community.

RMH DI eReferral forms can be accessed directly through provider Electronic Medical Records (EMR) using the eReferral button or by visiting the Ocean Healthmap website at www.oceanhealthmap.ca and searching for ‘Kawartha Lakes.’

What does it mean for CCCKL to be part of KL-OHT?

In 2019, Community Care City of Kawartha Lakes was proud to partner with the Ross Memorial Hospital to co-lead the start-up of the Kawartha Lakes OHT. We were inspired by the vision of a health care system that does a MUCH better job of connecting people to the care they need to both stay well and to recover from illness when it strikes. I know that individuals and their loved ones struggle to find the care they need when they need it.

In the 1980s and 1990s I was a Home Care Visiting Nurse. That often meant finding my way to homes in areas I was not familiar with. In those days there was no such thing as a GPS or “Navigation System.” Fast forward to today and those navigation systems are standard equipment in most new cars. But how does one navigate the health care system? The other day an individual shared his memories with me about when his mother-in-law experienced a stroke. He and the family didn’t know what to do. How did they take care of their mother-in-law now? What did they need to learn? Who could help them? They were bewildered and anxious. The individual was referred to CCCKL for home support and still recalls how relieved he was to find help. While I am grateful that CCCKL was there for this family, this situation should not happen. Instead, information and support should be available to individuals and families at the point of connection with the healthcare system—and even before that point of connection. The mandate and vision of the KL-OHT is to redesign the healthcare system so that individuals and families don’t have to worry and wonder how to meet their healthcare needs – from staying well to getting treatment.

CCCKL was founded by Valmay Barkey in 1985 with the central purpose of filling the gaps in the healthcare system and supporting individuals to live and thrive independently at home. For CCCKL, being a part of the KL-OHT means the opportunity to work with all of the KL-OHT partners at a system level to take the mystery out of navigating the healthcare system and ensure everyone has the directions they need for their healthcare journey.

What is CCCKL currently working on to improve the patient experience in our community?

A great question! I will start by explaining that while the term “patient” is often used in hospitals and treatment settings, at CCCKL we use the term “client” to convey our appreciation that individuals have a choice about where they access services and it is our privilege to serve them. Clients are at the heart of all we do at CCCKL, and that is why we use the “heart” as our logo.

There are many initiatives underway at CCCKL that aim to improve client experience. Firstly, we continue to meet regularly with our Client Advisory Council. Comprised of six residents of the City of Kawartha Lakes, Council members give CCCKL their advice and guidance on decisions about matters such as a new decal for our specialized transit vans, our Home Safety Risk Assessment and our Client Bill of Rights and Responsibilities. They also share their perspectives as clients who use some of our programs, helping us to provide the best service possible. Our Client Advisory members also give us input on the design and distribution of our Client Experience Surveys. Eliciting feedback from those who use our services is vital and we need to ask questions that touch on aspects of care that are relevant to our clients. I join with all of the management team in valuing and appreciating the input of our Client Advisory Councillors.

CCCKL is also excited to be implementing a new client database management system called AlayaCare. This new system will allow us to better serve our clients because information will be kept in a central place, accessible to all authorized staff members. This means that clients who are already using one community support service (e.g. transportation); and decide to use another service (e.g. Meals on Wheels) do not need to go through the intake process all over again—their information will already be in the system. We expect to “go live” with AlayaCare in September and are looking forward to a better experience for our clients.

A New Family Doctor for the City of Kawartha Lakes

City of Kawartha Lakes, June 15, 2022: Kawartha Lakes Health Care Initiative (KLHCI) is pleased to announce that Dr. Luke Johnson will begin practicing with the Kawartha North Family Health Team (KNFHT) at their Fenelon Falls clinic starting July 2022.

Dr. Johnson received his Bachelor of Science (Honours) from Carleton University (2009), his Bachelor of Medicine Bachelor of Surgery (MBBS) from the University of Wollongong, NSW, Australia (2013), and his Fellowship Royal Australian College of General Practitioner’s (FRACGP) in 2018.

Dr. Johnson and his wife, Claire, are the proud parents of two children. They are excited to be returning to Canada and especially to be settling in Fenelon Falls where their extended family has had a cottage for many years.

“It has been a pleasure to work with Dr. Johnson and Marina Hodson, Executive Director of the KNFHT, to facilitate this recruitment,” states Cindy Snider, Recruitment and Retention Coordinator for KLHCI.

We are pleased to announce that two new employees have joined the Kawartha Lakes Ontario Health Team this month. Please join us in welcoming Jackie Beckett and Sue Randell to the team! Read more about Jackie and Sue below.

Jackie Beckett – Decision Support Specialist

Jackie has a decade of experience working in multiple health care settings. She is a certified Health Information Management professional and holds a diploma from St Lawrence College in Health Information in addition to being a Laboratory Assistant. Her administrative experience in primary care settings and Electronic Medical Records is an asset to the work of the KL-OHT. Jackie has already played an extremely valuable role with the KL-OHT as the Implementation Specialist for the Online Appointment Booking initiative with all of the primary care providers in the KL-OHT.

As the Decision Support Specialist, Jackie will gather performance metrics from multiple health service providers to inform the priority projects and ongoing work of the OHT. This data analysis will guide us to impactful change initiatives in the patient’s journey and help care providers and patients/clients work to co-design system improvements.

Sue Randell – Quality Improvement Facilitator

Sue has an extensive background in healthcare, having worked in acute care, long term care and the mental health sector. She has specialized in Quality improvement facilitation and in accreditation activities. Sue has joined the KL-OHT as a part-time Quality Improvement Facilitator, and is looking forward to working with the team and all partners in our goal of making our patients’ journeys better.

As the Quality Improvement Facilitator, Sue will work with health service providers and patient/caregiver partners to explore and identify areas for improvement on our journey to a system of integrated care – beginning with the discharge process from hospital for older adults with chronic conditions.

How to Find a Family Doctor or Nurse Practitioner in Kawartha Lakes

The Kawartha Lakes Health Care Initiative is the lead organization for family doctor recruitment and retention in the City of Kawartha Lakes. Their vision is that all City of Kawartha Lakes citizens have access to a local family doctor.

The KLHCI has created guidance on how residents can find a primary care provider in Kawartha Lakes. Read on to learn how to find a family doctor in Kawartha Lakes.

How To Find a Family Doctor

If you are looking for a family doctor in the City of Kawartha Lakes and already have a doctor elsewhere in the province, follow Steps 1 – 3 . If you do not have a family doctor anywhere in Ontario, follow Step 3 .

Step One:

If you have a family doctor elsewhere than the City of Kawartha Lakes, have a discussion with them regarding your need to de-roster with them in order to register with Health Care Connect to access a family doctor here.
The majority of family doctors will continue to see you as their patient until you are connected here.

Step Two:

Contact Service Ontario at 1-888-218-9929
Have your Health Card available
Request to be de-rostered from your family doctor
Service Ontario will provide you with a letter in a few weeks stating that you are de-rostered

Step Three:

If you have completed Step 2, once you receive your letter you can then contact Health Care Connect to register
If you do not have a family doctor anywhere in Ontario, contact Health Care Connect to register

Community Paramedic Remote Patient Monitoring Program Update

Currently the team has 25 Remote Patient Monitoring devices, with 15 patients already using these devices to help support their health conditions including CHF, COPD, chronic hypertension, lung cancer, and more.

The Community Paramedic team continues to grow, with two new Community Paramedics recently joining the team. This will allow to a greater reach in our community and for more patients to be added to the program. The Community Paramedic team conducts pro-active, scheduled home visits, wellness assessments and phone calls to high need patients following discharge until the patient and family regain their ability to safely and confidently remain at home.

The Community Paramedics also play a vital role in offering the Remote Patient Monitoring program. This program enlists their expertise to provide health education as well as offering easy-to-use devices to monitor a patient’s health from the comfort of their home.

Currently the team has 25 Remote Patient Monitoring devices, with 15 patients already using these devices to help support their health conditions including CHF, COPD, chronic hypertension, lung cancer, and more. The rest of the devices will be rolled out to patients in need in the community, and an additional 25 devices will be added over the next month.

The majority of the patients already participating in the program are 80 years of age and older and many were initially worried they would not be able to use the equipment. After the Community Paramedic sits with them to set up the device and conducted some practice readings however, they love how easy it is to use. Daily readings from all patients to date have been successful at providing the needed health information.

A few of the patients of the Remote Monitoring Program have shared their thoughts on using the devices and how this has helped them monitor their health at home.

Chrystal is a Registered Practical Nurse and Personal Support Worker and joins us on a secondment from Community Care City of Kawartha Lakes. Chrystal has client and program management experience from her previous roles with CCCKL, including working on programs such as the Assisted Living Program, In-Home Respite, Home at Last and Supportive Housing. Most recently Chrystal became the Interim Program Manager for the Adult Day Program. As the Virtual Care Program Lead for the KL-OHT, Chrystal will be responsible for the providing virtual care navigation for the patients/clients participating in the program as well as increasing the overall capacity for delivery of virtual programming across the KL-OHT region. Susan Bacque – Experience Partner Engagement Lead

Susan is a seasoned public sector leader in quality improvement projects in health care, hospitals, and non-profit organizations serving caregivers. She has held senior positions in provincial and municipal governments focused on affordable housing solutions. She received two Reach for Excellence Awards as a provincial Director and was named a ‘Municipal Champion’ for her work in Peterborough. Susan holds an MA, a BSc, and is an accredited Municipal Manager. As the Experience Partner Engagement Lead, Susan will work closely with the Engagement Working group and Patient Family Advisory Council to ensure appropriate opportunities for residents’ involvement at key milestones in local health system transformation. Project ECHO: Epilepsy Across the Lifespan is a virtual case-based learning program which endeavors to educate healthcare practitioners caring for patients with epilepsy to enhance their care. The program is funded by the Ministry of Health of Ontario and provides CME credits at no cost. Sessions are 60 minutes and offered weekly over the lunch hour for ease of participation. Sessions begin with a quick 10-minute didactic talk delivering relevant clinical pearls followed by an interactive discussion of a de-identified real patient case brought forward by community healthcare providers. Our interdisciplinary teams located at academic hospitals connect with community practitioners via Zoom to deliver these sessions. There are three programs: Epilepsy in Children and Youth, Epilepsy and Adults and Epilepsy and Genetics. Those Family Practitioners, Paediatricians, and Community Neurologists who complete the Epilepsy and Genetics program will be able to order genetic testing for their eligible epilepsy patients through the Ontario Epilepsy Genetics Testing Program. There is rolling registration so no deadline to register by and the same sessions are offered on multiple days of the week and begin at various months throughout the year. (November 19, 2020) – Residents of the City of Kawartha Lakes are one step closer to experiencing more connected care in our community, as several health and service providers have been working together to form the Kawartha Lakes Ontario Health Team (KL-OHT). The KL-OHT received its official designation from the Ministry of Health on November 18, which signals that the team can move forward with plans to support a better patient experience, and support providers in delivering more efficient and effective care.

Key goals of the team include working together to reduce wait times between care settings, focusing on preventative health measures, and providing more virtual and digital health tools to help all residents, especially those in rural settings, access the care and services they need.

“We’re excited to reach the milestone of becoming a fully designated OHT, but most importantly, we’re excited to be able to get to work and create a connected health care system that puts patients first,” said Barb Mildon, CEO, Community Care City of Kawartha Lakes.

“Local physicians, patient partners and providers have united to f orm a strong partnership to co-design a seamless and integrated Kawartha Lakes healthcare system. Kawartha Lakes Ontario Health Team Submits Full Application to Ontario Ministry of Health

Lindsay, Ont. (September 21, 2020) – The Kawartha Lakes Ontario Health Team (KL-OHT) submitted its full application to become a designated Ontario Health Team (OHT) to the Ministry of Health on Friday, September 18, 2020. The KL-OHT was one of 17 OHTs provincially to be invited to submit a full application in July. The KL-OHT will learn whether it is an approved OHT by Monday, October 19, pending any unanticipated delays associated with COVID-19.

Ontario Health Teams are being introduced to provide a new way of organizing and delivering care that is more connected to patients in their local communities. Under OHTs, healthcare providers (including hospitals, doctors and home and community care providers) work as one coordinated team – no matter where they provide care.

The KL-OHT has made significant progress since it was invited to submit its full application on July 23. The KL-OHT has established working groups to enhance the patient and provider experience, including a digital working group with the aim of providing easily accessible virtual and digitally enabled care, and a care navigation working group that aims to break down existing siloes across all care sectors thereby streamlining how patients, clients, families and caregivers navigate through multiple care and service providers.

Additionally, KL-OHT partners worked together to develop the COVID-19 Assessment Centre, and collaborated throughout the pandemic on our community’s public testing strategy. The COVID-19 Assessment Centre has been successful in directing those requiring assessment and testing away from the emergency department and primary care offices. “Our OHT has already made positive headway in the community by connecting with patients and caregivers, as well as by providing a high-capacity, convenient COVID-19 assessment centre through a collaborative effort,” said Veronica Nelson, Vice President and Chief Operating Officer, Ross Memorial Hospital, and Co-Lead of the KL-OHT. “We are confident in our KL-OHT application and look forward to implementing the priority projects to provide connected care for our patients and clients.”

Lindsay, Ont. (July 23, 2020) – The Kawartha Lakes Ontario Health Team (KL-OHT) today announced that it has been invited to complete a full Ontario Health Team (OHT) application by the Ontario Ministry of Health. The KL-OHT was one of 17 OHTs provincially invited to submit a full application.

“We are excited to take the next step toward full implementation of the Kawartha Lakes Ontario Health Team, and toward a local healthcare system that puts the patient first through an integrated approach,” said Veronica Nelson, Vice President and Chief Operating Officer, Ross Memorial Hospital, and co-lead of the KL-OHT. “Working closely with our partners, we look forward to connecting local care for our community.”

This next step in the OHT application process follows a successful progress report submission on January 20, prepared in consultation with many community and patient/caregiver partners. This progress report demonstrated the progress the Team has made in patient/client partnerships and community engagement, patient care and experience, digital health, governance, and quality improvement.

The full application is due on Friday, September 18, with results expected back by Monday, October 19.

In addition to the invitation to submit a full application, KL-OHT is pleased to announce its official name, logo and website to the public. The website, http://www.kawarthalakesoht.ca serves as a hub for the public to receive updates on the OHT’s progress, news relating to the health care industry, and to connect with the Kawartha Lakes OHT.

Along with the logo and website, the Kawartha Lakes OHT also unveiled a new tagline – Connecting Local Care. The tagline emphasizes the purpose of the OHT model: to provide a patient-centric healthcare experience that ensures all local services work together to ensure the well-being of all patients, from first point of contact through their entire healthcare journey.

Progress report marks the next step toward submitting a full application, designation as approved OHT

(January 27, 2020) – The City of Kawartha Lakes Ontario Health Team (CKL-OHT) has taken the next step toward designation as an Ontario Health Team by submitting an official progress report to the Ontario Ministry of Health. The report, created in consultation with the many community and patient/caregiver partners, details the Health Team’s readiness to submit a full application.

The progress report serves as an addendum to CKL-OHT’s previously submitted self-assessment and demonstrates the progress the Team has made in patient/client partnerships and community engagement, patient care and experience, digital health, governance, and quality improvement since July 2019 when it was designated as “in development” by the Ministry.

CKL-OHT recently held advisory sessions to ensure that the voices of patients/clients and families were reflected in plans to enhance our local healthcare delivery. The team will host public engagement sessions throughout 2020 across the City of Kawartha Lakes, to seek broader input about the healthcare gaps and challenges experienced in our communities, particularly when it comes to aging and transitioning between home, hospital and community care.

“We’ve partnered with several local healthcare organizations and will continue to look for opportunities to work together with community, health and service providers as we shape our team,” said Veronica Nelson, Interim CEO and President, Ross Memorial Hospital and Co-Lead of CKL-OHT. “Together, we are determined to create a connected and supportive system in the City of Kawartha Lakes.”

Year One Focus

CKL- OHT will focus initially on enhancing coordinated care and services to a target population that includes seniors 65 years and older who live alone or have a poor support network; who are frail, with complex medical needs or are cognitively unwell; and who have been hospitalized within the last year. As the work of the OHT evolves, the target population will expand.
Public engagement sessions will be available on the new CKL-OHT website, launching in the near future. The website will also offer a survey tool for patients, families and service providers to share their feedback.

Alderville First Nation's Health and Social Services Department offers a variety of programs to promote community well-being. Weekly activities include access to a fitness room, fitness classes, Meals on Wheels, youth nights, Ojibwe singing lessons, Tae Kwon Do classes, and a community care program providing home maintenance assistance for elderly and disabled members. Monthly offerings feature support groups for diabetes and cancer, a doctor's clinic, dietitian consultations, foot care clinics, an elders' program, a parent support group, and a traditional medicine clinic. Annual events encompass Family Wellness Week during March Break, an October Health Fair, Men's Night, and an Elders' Dinner.

Submission to form CKL Ontario Health Team enters next phase of readiness

Lindsay, Ont. (July 25, 2019) — A submission to form a City of Kawartha Lakes Ontario Health Team (OHT) to better coordinate care and services in the City of Kawartha Lakes has progressed to the status of ‘In Development’ as announced by the Ministry of Health on July 18th.

It is the provincial government’s intention to better connect the health care system through the phased implementation of Ontario Health Teams. Ontario Health Teams are groups of providers and organizations that are clinically and fiscally accountable for delivering a full and coordinated continuum of care to a defined geographic population. The designation of ‘In Development’ means that the Ministry will provide additional resources as the partners further develop their submission.

Local health care organizations discuss formation of Ontario Health Team

Lindsay, Ont. (May 7, 2019) — Several healthcare organizations in the City of Kawartha Lakes are discussing the formation of a local Ontario Health Team (OHT) to better coordinate care and services for local residents.

On February 26, the Ministry of Health & Long Term Care announced its intention to better connect the health care system by implementing Ontario Health Teams. Ontario Health Teams are groups of providers and organizations that are clinically and fiscally accountable for delivering a full and coordinated continuum of care to a defined geographic population. Our geographic population lives within the City of Kawartha Lakes (CKL).

At present, the organizations that are preparing a joint self-assessment to potentially form a local OHT include Community Care City of Kawartha Lakes, Kawartha North Family Health Team, Canadian Mental Health Association Haliburton Kawartha Pine Ridge, Extendicare Kawartha Lakes and Lakeland Village, and Ross Memorial Hospital.

These organizations have long partnered to serve their shared communities. They currently run joint programs and services under various formal agreements and Memoranda of Understanding. If approved, the formation of this OHT would build on these existing partnerships to provide seamless and coordinated care for our CKL residents.

If the group’s self-assessment is approved, the OHT will focus initially on enhancing coordinated care and services to a target population that includes seniors over 65 years who live alone or have a poor support network; who are frail, with complex medical needs or mental health or addictions issues who find it hard to navigate the healthcare system; and who have been hospitalized within the last year. As the work of the OHT evolves, the scope of care and services as well as the patient/client/resident focus would expand.

The self-assessment to form the local OHT will be submitted by the provincial deadline of May 15. The Ministry of Health & Long Term Care will be inviting those that are ready to submit a detailed application. The first phase of OHT implementation is planned for this fall.

Feedback will be welcome throughout this process. Please watch for opportunities to provide feedback, share suggestions, and identify current gaps in services.
